Blackwood Recreation Centre would like to extend an opportunity for those participants who have been identified as a Close Contact, Classroom Contact or confirmed as a Positive Case to request a credit for missed classes during a mandated quarantine in line with Government Directions. Please read each question carefully and ensure you fill out the entire form to be eligible for a credit.
